On October 30, 1984, Chicago Fire Department Firefighter Phillip Kenneally of Hook and Ladder Company 38 died in the line of duty during a fire in a three-story apartment building. Kenneally and other firefighters were on the roof of the building to cut ventilation holes, but he became disoriented when the smoke became extremely thick, and he fell down a three story airshaft. Kenneally was taken to Swedish Covenant Hospital where he died the following day from his injuries.
